What to expect financially at the time your visit:
- Please bring your insurance card to every visit for verification
- We collect all patient responsibility (copays, deductibles, and coinsurances) at Check-In for all procedures and office visits.
- We can give you an estimate of what you will be expected to pay, if you would like an estimate please call the billing department.
- Additional procedures performed during your visit may include an additional cost to the patient. Additional charges will be billed to the patient and billing for specimen testing will be completed by CBL path.
- Any patient with a past balance will be asked to pay the balance and the estimated responsibility at check in
- The office accepts, VISA, MasterCard, cash, and check (checks are not accepted for new patients or cosmetics)
