To access your records, please follow these steps:
  1. Go to tangoren.ema.md or select the "Modernizing Medicine" icon above
  2. Enter your user name. Your user name is the first 2 letters of your first name, the first two letters of your last name, and the month and day of your birthday.
    Example: John Smith, DOB 02/01/1999, username = josm0201
  3. Enter your password. If you have not created a password an email with a temporary password good for 24 hours would have been emailed to you after your last viist.
To access your records, please follow these steps:
  1. Go to portal.advancedmd.com/account/logon?lk=120837 or select the "AdvancedMD" icon above
  2. Your user name is your email address as given to the office.
    If you have not registered, you will have to first register.

    When registering for the first time do not include the last four of your social security number.



If you are having difficulty signing into either portal, please call the office at 315-424-1430 ext 5. We will be able to send you a new invitation email.

Please contact the office directly by phone for all urgent matters (ie worseining of an illness). Portal messages will be answered within two (2) business days. Some information will not be reviewed / updated until your next scheduled visit in the office.

As of March 1st, 2013, we are asking all patients to fill out our new paperwork. Please take the time to fill out these new forms and bring them with you to the appointment. Patients that have had appointments before March 1, 2013 are being asked to fill out our new patient paperwork found below. This paperwork allows the office to remain HIPAA compliant and better serve you as a patient.
